Future Begins at Home Prevention/Diversion Program

Family of Tulsa County has been awarded a very limited amount of funds to assist with eviction prevention and shelter diversion. Not all applicants with qualify. Upon submission, if we are able to assist you, you will be contacted within two business days. If we do not reach out, we are unfortunately unable to assist you at this time. Please reach out to 211 for additional resources.

Applications are ONLINE ONLY. Please do not call or email. 

FORM MUST BE COMPLETED IN FULL TO BE CONSIDERED. ANY APPLICATION THAT IS NOT COMPLETE WILL BE DENIED.  THERE HAS TO BE AT LEAST ONE CHILD UNDER THE AGE OF 18 LIVING IN THE HOME TO QUALIFY FOR FUNDING.

Applications will be reviewed in the order they are received. Please do not call or email.  If you meet the qualifications someone will contact you within two business days to verify information. A return phone call or email does not guarantee funding.

You will be required to provide the below documentation within one business day, if requested, for review. If the information is not received upon request your application will be denied. 

Required Documents:
• Photo ID
• Valid lease in applicant's name
• 5 Day notice to pay eviction notice with amount owed
• If child is not on the lease, we will require a birth record or other form of ID.
• Proof of income 
• Other documents as requested 
 

Eviction Prevention

One-time financial services for families at imminent risk of homelessness. Funds will prevent loss of home and stabilize current household situation.

Qualifications:

•Must have at least one child under the age of 18 living in the home.

•Family is on a lease in the unit.

•Eviction process has begun

• MUST be able to contribute to the outstanding balance

Shelter Diversion

One-time financial services for families at imminent risk of homelessness. Funds will assist with utilities or move in costs.

Qualifications:

•Must have at least one child under the age of 18 living in the home.

•Bill is in the applicants name

•Cut off notice has been received

•Approved for new housing

• MUST be able to contribute to the outstanding balance or move in costs

Our prevention/diversion program has limited funds, therefore, we are unable to offer financial
assistance to every family that applies.  If we are able to assist you, we will contact you within
48 hours and schedule an interview. If you have not received a reply within 48 hours we are unfortunately unable to approve your application. Please contact 211 for additional resources.
